We offer serivces to women and men of reproductive age through the Federal Title X program. All services are confidential.

- Pap Screening
- Contraceptives, including Emergency Contraception
- Affordable Fees, many on a sliding fee scale based on income
- Education and Counseling
- Referrals to other Providers
- Sexually Transmitted Disease Testing and Counseling

Family Planning Forms:
Forms should be completed prior to appointment, so please come 15 minutes early to complete your registration, medical history, and other forms.
Please be sure to bring either a current pay stub or your tax return for verification of income.
Some testing and services may not be available on the sliding fee scale. We are happy to work on a payment plan for you if you are not able to pay in full.
